Scenario and Task

October, 2001
You are a family doctor who has a healthy 60 yr. old man (John Androman) come into your office requesting to be given replacement hormone therapy (testosterone) so he can be more youthful and enjoy an active upcoming retirement.

Develop Assessment Criteria

As this man=s physician you have been entrusted with properly assessing this man=s health and determining whether prescribing hormone therapy is a warranted course of action.

John Androman=s Medical History

A recent physical examination including blood work was done on John and every aspect of his health was well within normal levels for his age

A testosterone level test yielded a result of (450 ng/dl) 

Past History

-kidney stone when he was 35 (treated and has not reoccurred)

-tendonitis in his right elbow 2 years ago due to playing too much golf (with a poor swing) (treated and has not reoccurred.... he took golf lessons to help him)

Analyze data and information

1.a)     1. a) What are steroid hormones?
b) Write down the molecular formula for testosterone and draw the molecular structure for testosterone. What elements make up this molecule?
c) What type of molecule are steroid hormones like testosterone (carbohydrate, lipid, protein, other)?
d) How do steroid hormones find the proper cells in your body to work on?
e) How do steroid hormones get through the cell membrane?
f) What part of the cell do steroid hormones act on?
g) When testosterone enters a skeletal muscle cell it promotes synthesis of proteins which will affect muscular strength. Describe where and how proteins are made inside a cell.

2. a) John has a testosterone level of 450 ng/dl in his blood. Is this considered a normal level (LINK#7) for a man his age?
b) According to the web based company reNEWMAN, this is misleading. Why?

3. a) Use information from  the reNEWMAN
treatment Program 1 (assuming 8 IU's of growth hormone are used a week, and two checkups are done each year), calculate the total cost (in Canadian dollars, converted from American dollars) for John's request for hormone replacement therapy.

Propose Course of Action

4. Write up a risk / benefit analysis to help you decide whether you should recommend John should or should not pursue hormone therapy.

5. Write a summary of your findings and what you have decided to discuss with John regarding his request for hormone therapy. Support your decision with the information you have learned.

6. What do you feel is the driving force behind the (ab)use of steroids by people who do not need them for medical reasons?


7. What might happen to society if steroid use was not controlled? Relate your answer to; athletes (both men and women), use by teens, and as in this case older men.
